FREQUENTLY ASKED QUESTIONS ABOUT DAKOTA COMMUNITIES

Do you only provide services in Dakota County?

We provide services throughout the seven county Twin Cities metro area – including Anoka, Hennepin, Ramsey, and Dakota, Isanti, Sherbourne, and Wright counties.

 

Are you an affiliate of Dakota County?

Established in 1972, we are an independent non-profit organization providing services to people with disabilities.  Our original name was Dakota’s Children.  Our name changed to Dakota Communities in 1997 to reflect the broadening of our service provision.

 

What services do you provide?

We provide Residential (Waiver, ICF), In Home Family Support, Semi-Independent Living Services (SILS), Independent Living Services (ILS), Personal Support, Supported Employment Services (SES), and Respite. We also provide consultation for Therapy, Behavior, and Therapeutic Recreation.

 

Where are your homes located?

Robbinsdale, Crystal, Maple Grove, Plymouth, Brooklyn Park, Brooklyn Center, Mendota Heights, Inver Grove Heights, South St. Paul, West St. Paul, Eagan, Lakeville, Burnsville, Little Canada, Maplewood, Cambridge, Buffalo, Big Lake

 

What funding sources do you accept for In Home services?

We accept County Funding including waivers (MR/RC, CADI, TBI), Consumer Support Grants, and Consumer Directed Community Supports.  We also accept private pay.

 

Do you have a waiting list for your group homes?

While we do not keep a waiting list for future openings in our residential sites, we are always happy to discuss placement in our homes and keep in contact with people interested. Openings are listed on our website.

 

Do you work with people with Autism?

Yes, we provide services to people with diagnosis on the Autism Spectrum throughout our residential settings and in our SILS, In Home, and ILS services.  While we do not have a home specifically for people with the Autism diagnosis,  we work to ensure that individuals with similar needs are together and that their direct care staff are trained in the specific needs of the person they support.

 

Do you work with people with TBI?

Yes, we provide services to people with TBI diagnosis through out our residential settings and in our SILS, In Home, and ILS services.  While we do not have a home specifically for people with the TBI diagnosis, we work to ensure that individuals with similar needs are together and that their direct care staff are trained in the specific needs of the person they support.

 

Do you provide weekend respite?

While we do not currently provide a weekend respite program, we have two respite beds – one in West St. Paul and one in Lakeville (MS Foster Care).   On a limited basis and depending on individual needs and our resource availability, we may also be able to provide In Home Respite.

 

What is the intake process for In Home Services?

Please contact your representative or case manager to begin a conversation with a Dakota Communities Service Coordinator who will complete an initial intake interview by phone or email.  Once funding and staffing availability is determined, a formal intake meeting will be scheduled with a Dakota Communities Service Coordinator, the person requiring services, and their “team” of support people to make specific service plans.

 

Do you have a “pool” of staff for providing In Home services?

In-home care is provided by Dakota Communities’ trained and experienced employees who currently work In Home hours or in our group homes.  We work to ensure that the direct care staff provided for In Home services is a good match for the person and their support needs.  We ensure that each direct care staff is trained specifically for the people to whom they provide support.
